Please send ONE of the following types of specimens:

• EDTA whole blood (purple top): minimum of 6 ml
• Fibroblasts, amniocytes, or CVS: 4 confluent T-25 flasks
• Extracted DNA

General Instructions:

• Please fill out Form 1 and Form 2 completely and include with the specimen.
  
• Whole blood should be stored at 4°C until shipped.
• Confluent cells should be shipped in T-25 flasks filled to capacity with cell culture media.
• Samples can be shipped at ambient temperature via an overnight courier.
• Deliveries cannot be accepted on weekends or holidays.
