Payment by Institution

• CTGT will bill the institution requesting the test for the entire cost of the test.

Payment by Insurance - CTGT will only bill insurance companies directly when all
of the following criteria are met.

• CTGT is not a networked provider with any insurance company, and we reserve the right
  not to bill selective insurance companies.
• Credit card information must be supplied by the patient for any outstanding balances not covered
  by insurance, up to the full amount of the testing ordered.
• All information in the “Insurance Billing” section of Requisition Form I must be completed, signature of insured is required.
• The patient will be billed for any amounts not covered by insurance, up to the entire cost of testing.
• Pre-authorization must be obtained prior to sending sample to CTGT. Authorization or letter of agreement must:
  • be directed to CTGT. Preauthorization directed to the submitting facility or physician is not acceptable.
  • detail the correct re-imbursement rate and number of CPT codes for the testing ordered.
  • detail the start and end date during which services are covered.
• A clear copy of both the front and back of the patient's insurance card.

Payment by Patient

• The patient may pre-pay for the requested test.

• Mastercard or Visa
