After you've found the test(s) you'd like, please follow these steps to order it.
1. Complete Requisition Form

2. Collect Specimen
3. Ship Order to CTGT
Please also be sure to include a packing slip with your shipment.
4. Receive Results
Results will be faxed and mailed to you at the contact points you provided on the order form.
